Requirements for Forming an LLC 
 Below is the necessary information you?ll need to know to complete the online Limited Liability Formation (LLC) application process.
  •  The state where you will be forming the LLC.
  • A preferred and alternate entity name for the LLC.
  • The name, address, and contact information of an individual over the age of majority that our office can contact regarding the processing of the LLC.  
  • The official business address of the LLC at which location it will be conducting business.
  • The name and address information of an individual which you must designate as the registered agent of the LLC to accept service of process on behalf of the LLC. The agent must be an individual over the age of majority and must maintain a physical address within the state of LLC formation. (NO P.O. BOX OR PMB?S ALLOWED!)
  • The name and address information of each initial member of the LLC (Minimum of 1 Member required).
  • Each initial member?s contribution and percentage of ownership within the LLC.
  • The name of each individual who will be serving as the President, Vice President, Secretary, and Treasurer of the Corporation. (NOTE: ONE PERSON may take all officer positions in a ONE PERSON Corporation).
  • The type of business activity which the Corporation will be engaging in.
  • Method of Payment.

Below is the necessary information you?ll need to know to complete the online Incorporation application process.

  • The state where you will be forming the Corporation.  
  • The type of Corporation which you will be forming.
  • A preferred and alternate entity name for the Corporation.
  • The name, address, and contact information of an individual over the age of majority that our office can contact regarding the processing of the Corporation.
  • The official business address of the Corporation at which location it would be conducting business.
  • The name and address information of an individual which you must designate as the registered agent of the Corporation to accept service of process on behalf of the Corporation. The agent must be an individual over the age of majority and must maintain a physical address within the state of Incorporation. (NO P.O. BOX OR PMB?S ALLOWED!)

What is a DBA?

 A "DBA" ("Doing Business As") is an official and public registration of a business name. The business may be a sole proprietorship, partnership, corporation, limited liability company (LLC), or any other type of business entity. DBAs are also known as Fictitious Business Names, Assumed Names, and Trade Names. DBAs are most commonly filed for one of two purposes: to register a business name for a non-state-level business entity (such as a sole proprietorship) for purposes of conducting business, marketing and/or banking; or to register an alternate name for a state-level entity (such as a corporation or LLC) that intends to conduct business under names other than or in addition to its legal name. In many cases DBA registrations are required by law or by banking institutions. Even when not required by law, anytime an individual or company is using a business name in commerce it is wise to file a DBA.
